Highly Qualified Teacher (HQT)
Forms for ESEA Highly Qualified Teacher (HQT) Status Determinations
Forms to be completed for all teachers teaching core academic subject classes and for any teacher where a change in teaching assignment or where changes to the teacher’s HQT status may exist
- For Teachers with Less Than One Year Experience, Teaching Elementary (K-8) or Secondary (9-12) Level
- For Teachers with One or More Years of Experience, Elementary (K-8) Level
